This book was bought in 1980 for $29.95 and (evidently) NEVER EVER "cracked open" - though it has LOTS of (bore-ing) metrics, charts, tables, and graphs, and (then-useful) appendices contributed by others. UR (utilization review) seems to be important in this book.
This is a really comprehensive guide to all stages of designing and fielding survey research. While the explicit focus is health, and the examples given all relate to health, the methodological information is really of use to anyone interested in survey research in any field.
