This volume is lovingly and gratefully dedicated to: My Dad, Bob Taylor, whose serious illness was discovered during the time of this writing, and whose courage was a deeper motivation for me to do this work as quickly as possible. He knew the book would be dedicated to him, and we both had hoped he would live to see it finished. In God's greater wisdom Dad was taken to heaven in the midst of my work on this book.
